1. Internal audits have become a tick-box exercise

If audits feel repetitive or are just “done for the certificate,” it’s time to rethink. Internal audits should highlight what’s changed, what’s working, and where improvements can actually add value.



2. Objectives haven’t been updated (or achieved) for a while

When objectives stay the same year after year, it usually means the system isn’t being used to drive improvement. Your ISO framework should evolve with your business.



3. No one’s sure who owns what anymore

If responsibilities are unclear, updates go missing, or documents aren’t maintained, it’s a sign your structure needs a refresh, especially after staff changes or growth.



4. NCRs and opportunities for improvement are ignored

Nonconformities and/or audit findings are not failures, they’re feedback. If issues get logged but not discussed, you’re missing one of the biggest benefits of having a management system at all.



5. The external audit feels stressful

If you’re scrambling for records every year, something’s off. A well-maintained system should make surveillance audits almost effortless.
